How to Enroll
If you are ready to enroll in the WellSpan Retirement Savings Plan, all the materials you need are available to you on this Web site. You are encouraged to use DreamTracker^SM to help you determine how much you want to contribute and review the Enrollment Guide to learn what might be the right investment choices for your individual needs.

Once you've done that, follow the steps below:

1. Complete and return the Salary Reduction Agreement form
• Salary Reduction Agreement form
2. Complete and return the Enrollment form
• 403(b) Enrollment form
• 401(k) Enrollment form
3. Complete and return the Beneficiary form
• 403(b) Beneficiary form
• 401(k) Beneficiary form

All forms should be returned to Katrina Boucher, our onsite benefits consultant, in person or via mail to the following address:

And if you return the forms by mail, keep a copy for your records and verify your upcoming paychecks to ensure that your contributions to the Plan have begun.
